  • There is NO maximum limit on your out-of-pocket expenses.
  • No out-of-network coverage
  • You must live and reside within the H.M.O. service area.
  • Moving could result in loss of coverage.

    H.S.A.'s combine a high deductible major medical policy with a Tax-Favored Savings Account
  • Savings account: "save up" deductible
  • Savings account: deposits accumulate tax free, Interest earned is tax free
  • All medical withdrawals are tax free and account balance rolls over year to year.
  • Underlying major medical policy can be either PPO or Non-PPO.

PPO Co-Pay Plans

  • Inpatient
  • Usually no co-pay for hospitalization.
  • Deductible & Co-insurance amounts are owed to hospital, doctors, and others.
  • Arrangement to pay hospital can usually be paid over time.  
  • Outpatient
  • Co-pays range from $10 to $100.
  • Co-pays are for each service each time.
  • Co-pays DO NOT apply to deductible.
  • All things being equal, the lower the WPG, the higher the premium.

  • Hospital bills discounted.
  • Discount applies to all covered expenses.
  • All covered expenses apply to deductible and co-insurance.

  • Discounted ranging from $5+.
  • Discounted payments are for all services until deductible is met.
  • All covered payments apply to deductible and co-insurance.
  • All things being equal, discount plans are less expensive than co-pay
